Airline Pilot Magazine propagates this by naming all their members either Captain or F/O... even in death in their memoriam section. So for instance F/O Smithwicks passes away and is remembered as F/O Smithwicks... but this was probably his last job, as Smithwicks likely was Captain Smithwicks at s...
