OK Pygmie and Jerricho. I think I understand what you are getting at now. What you are saying is that when approach clearance is issued, if it is not stated, any restrictions in place are cancelled. If said restrictions were requested by the tower controller you would re-state them with the approach...

These altitude restrictions you are talking about are specified in local agreements and alike. If you are cleared for a visual approach by a terminal controller, YOU ARE NOT required to adhere to a previously issued altitude unless it it specified again. Excellent equipment, fair pay; pilot's are expected to perform some non-flying duties but this is typical in a smaller operation.
