People are responsible for safety in all levels of aviation. The only thing that happened here was that holes in the slices of Swiss cheese lined up. Why do you have such a hard-on for the CP?

I don't know about overriding the pilots. But I don't see why there couldn't be a land/don't land calculation for landing much as V1 is for a balanced field takeoff. Might keep the runway available in the minds of the pilots during high workload landings. There is a hesitation to performing a go-aro...

Hate to say it but: This sort of accident is just waiting to happen again. With the job market the way it is the experience drain is really showing in the entry and mid levels of the industry. I couldn't help but be alarmed at the total IFR experience between the PIC and the FO... it was around 500 ...

Initial training flight times (I think)
Beaver: 3 hours
Goose: 10 hours, 50 hours line indoc
Beech 200, 1900: 5 hours, 20 hours line indoc
Saab 340, Shorts 360: 12 hours pf, 12 pnf sim (typically) , 3 t/o in the plane and 50 hours line indoc.

My dad saw floating spots and it ended up being a detached retina requiring surgery. I think I would consult an optometrist rather than trust my fellow Avcanadians to diagnose my medical problems. Good luck.
