Liard Air Twin Otter Accident 2007
It's been nearly a year since this tragic accident and I can't seem to find any more information on causes. Was it not investigated since the aircraft was destroyed by fire?

Warning: The link I posted has a video which is of the actual crash in which someone lost their life. Might not be suitable for all viewers.

Re: Liard Air Twin Otter Accident 2007
Looking at the video it sure was shortly after take off, it was barely airborne when the wing hit the ground....Narrative:
The Twin Otter crashed shortly after takeoff from the gravel airstrip at Muncho Lake. A post-impact fire erupted and the airplane was destroyed.
The hardest thing about flying is knowing when to say no
After over a half a century of flying no one ever died because of my decision not to fly.

Information from the CADORS:
Some speculation posted here: AvCanada: Munch Lake Crash
So, that is the occurrence number to keep an eye out for. It takes quite some time for reports to be published on the TSB website once they are complete. However, it does not appear that this report has yet been completed.UPDATE / Add Info from TSB: A07W0128: Added Occurrence No. and Class of Investigation (Class 3).

There was some discussion about this accident in "The Indispensable Otter" thread. I said I had asked for the TSB report, and post when received ... so here it is, attached.

Thanks so much for finding that Widow.
Definitely good reading for those DHC-6 operators using short strips. Maximum Performance STOL operations require specific training (including flight simulator), auto-feather (Remember your taking off below VMC), and some other considerations including proper planning using the performance charts. Charts don't take everything in the real world into account though. Runway slope, and different surface conditions, etc. can have a big effect. Small things like straps, survival equipment, tools, granny gas, etc. can add up.
Widow, can you request the report for this accident?:

Hi Rudy,
I did a quick google in order to find the TSB report number and have requested the report. FYI, reports under 20 years old can be requested from this page: http://www.tsb.gc.ca/eng/contactez-contact/index.asp.

Re: Liard Air Twin Otter Accident 2007
I have received copies of older reports going back more than 20 years as well ... at least to PWA 314 at Cranbrook in 1978. A lot of the reports from the CASB days were published in journal format, so they may be more difficult to retrieve as there were a bunch combined into a single document.
