USA border crossing procedures

its been a while since i have flown to the usa. can someone refresh me on what is required to get that customs sticker thingy put onto the plane? also where can i find out info on the paperwork required to fly across the border?..

Get the decal
Enroll in Eapis to get a user name and password, you need to file both entering and leaving the Good Ole USA
Log into Eapis and create an itinerary with your travel info
Call US customs at your intended port of entry and advise of your arrival time
File flight plan with Nav Can to get code
Take off, squawk code, radio (flight service) and close flight plan before you land.
Arrive on time, have all your docs and be nice to the customs man.

Reverse the customs and flight plan people and do the same steps to return.
