I live in Ontario, Canada. I just had my form signed by my family doctor but she wasn't sure where to send it so she made it my responsibility.
Sorry if this is an obvious question but there are so many emails online I don't want to send it to the wrong one because I know there's a 1-3 month wait to get it back.
If anyone has some info that would be much appreciated thanks.

Nooo… don’t send a Category 4 declaration to the medical office. It goes to the regular contact email which is aviation.ont@tc.gc.ca
