Ok came across this on Airliners.net about pilots smoking. Scroll down to a post by "September11" he has a picture of an AC DC-9 from 1983 smoking on the runway in Kentucky. He says it was caused by smoking a ciggarette.

Correct me if im wrong wasnt this caused by a faulty lav pump that was reset?
