Application for Aviation Document

Hi, I am in the process of filling out the application for an aviation document, and it requires me to send in a passport style picture, but I am wondering if I can get it certified by a parent, because it requires a signature on the back?

A family member or any individual residing at your address may be your guarantor provided he or she meets the specified requirements.

Thanks, I also have one more question, my class 1 medical expired, and it is asking for the medical category, so should I just put 3 or leave it at 1?

This is for the ppl by the way

You have PPL privileges of a 3 when the 1 year is up (assuming you're under 60), but the document is still technically a cat 1 with restrictions. Also, the form asks for the last renewal date, so TC will take it from there.
