I've never put the oil cap back on slightly out of whack and had a tail boom covered in it... never.

Yep, same here....on my commercial flight test no less. Hasn't happened since

I left an R44 fuel cap behind. Only once though.

Speaking of mess up's, the DAY AFTER the training pilot in my first rotary job finished me up, he took off in an L3 with the wheels still on.... Ooops.

That was the most retarded thing I have seen in a long time! A helicopter in a stadium full of people! Did you notice that the guy with the hat raised his arm to get it even closer to the rotor? He should have jumped up and down also!
