Call one of the local flight schools, they should have an Industry Canada delegated examiner.

For replacements, email your nearest Spectrum Management / Industry Canada District Office.
In my case, it was Toronto - their email is spectrum.toronto [at] ic.gc.ca
I provided them my full name, DOB, mailing address, and when the certificate was originally issued. They were really fast at sending my new one.
