I have a question (or two) in regards to CADORS.
Two examples:

In the last box "Detailed Information" there is a section "further action required"
My question is - What triggers the need for further action?
In the above examples one example has two excerpts. The first "Detailed Information" didn't trigger "further action" the second did ! Why?
In the other example why would two missed approaches require further action. Did the flight crew not do what they should have? Why would two missed approaches even trigger a CADORS?
Anyone?

Cause only one triggers it. You're closer than 200ft from rwy with TC pick-up truck...CADORS. You do a right downwind circuit but it should had been left....CADORS. You overshoot for whatever...CADORS. You don't call down and clear at a RCO..... CADORS.
The Best safety device in any aircarft is a well-paid crew.

The name says it all anything that happens that it out of the ordinary, eg an aircraft going missed for reasons other than weather is out of the ordinary and not efficient so a CADOR is put out. Just because something has a CADOR written about it doesn't mean someone screwed up it just means something out of the ordinary happened its kinda like the idea behind SMS if everything out of the ordinary or with a potential for risk is reported patterns will start to emerge and it will give TC or your company a better way to see what area's need to be focused on to help mitigate risk or increase efficiency.
