C-ICTC, a Quad City Challenger II advanced ultra-light aircraft, took off from runway 02 at
Manitowaning/Manitoulin East Municipal airport, ON. Video of the take-off shows that as soon as
the aircraft became airborne the pilot had difficulty controlling the aircraft and the engine began to
surge. The pilot attempted to return to land on runway 02, but aerodynamically stalled while
attempting to maneuver at low altitude. The aircraft crashed near the windsock and main ramp
area in a ditch. The pilot was not injured, but the aircraft sustained substantial damage.

C-IFKS, a privately owned SONEX advanced ultra-light, was conducting a visual flight rules flight
from Kingston, Ontario to Westport Rideau Lakes (CRL2), Ontario. On landing, the aircraft
bounced and drifted laterally away from the centerline to the left side of the runway. The aircraft
port wing contacted tall grass which aggravated the drift and resulted in a runway excursion to the
left. The aircraft rolled into the long grass and as it was slowing, cartwheeled, overturned and came
to rest upside down. The pilot and the passenger egressed the aircraft safely. The aircraft received
substantial damage. The ELT activated; there was no post impact fire. The aircraft was recovered
and stored in a local hangar. JRCC Trenton dispatched a CH146 Griffon helicopter to locate the
ELT signal. The CH146 landed at CRL2, and crew members found and turned the ELT off. Medical
assessment indicates the passenger received serious injuries.

local flight. While airborne, the pilot suddenly heard a loud noise emanating from the engine, which
was followed by a loss of power. At that moment, the aircraft was flying over Lake Belwood, near
Fergus, Ontario, at an altitude of approximately 900 feet AGL. Consequently, the pilot made a
forced landing on the water. Shortly after touching down, the aircraft began to sink. The pilot
egressed the aircraft and was later rescued by local residents. The pilot sustained only minor
injuries and was taken to the hospital for evaluation.

multi leg flight from Chetwynd (CYCQ), BC, with only the pilot on board. The aircraft was reported
overdue at approximately 1715 Pacific Daylight Time (PDT) and a search began. The aircraft was
found approximately 37 nautical miles west-southwest of CYCQ, near the shore of Simpson Lake
(55° 32' 05" N, 122° 40' 24" W), BC, with substantial damage. The pilot was fatally injured. RCMP
responded.
The TSB was notified at 1811 PDT by Joint Rescue Coordination Centre in Victoria, BC.
