Dash 8 Departs with insufficient fuel

This is not a shot at Perimeter Aviation but a interesting read how people deviate from SOP's and set themselves up for a serious event. Fortunately it was just an embarrassing event :oops:

Synopsis:
The deHavilland DHC-8-102 departed from Winnipeg/James A. Richardson International Airport, under instrument flight rules for a scheduled flight to Island Lake, Manitoba. At about 1250 Central Daylight Time, during the climb check, the crew determined that the fuel quantity was insufficient for the intended flight. The flight returned to Winnipeg and landed at 1306 without further incident.

I'd have to say good job to the crew for actually turning around when the probably just have just "risked it" and made it to Island Lake without anyone being the wiser. Still a mistake but at least they did the right thing

I agree with Fish4Life, the crew did the right thing by going back.

Mistakes happen, but at least this crew was honest enough to do the right thing as soon as they discovered the mistake.

I am hesitant to say good job, but I think we should let it just go. The crew eventially did the right thing by returning, and if crews start getting slagged for admitting their mistakes, the next thing we will see is a crew doing something like this and trying to make it...I have seen similar in the past with fuel issues. And killing a bunch of people because you were afraid of the consequences of screwing up is not a good option.

We all talk of pressure being put on pilots. Look at how TC responded to this one, and ask yourselves if TC should not bear some responsability for putting pressure on pilots to cover up.

You make a mistake. Nobody hurt. Nothing damaged. You admit it..Corrective action is taken to ensure it does not repeat itself...I fail to see why TC need to be monitoring etc...
Surprises me actually that the Captain was not violated

Anyway, no foul here and I dont think it will happen again at perimeter, or hopefully anywhere else

Here is what happen when you screw up with your fuel and don't turn back or divert in time.
US military Dash-8 crash landed near Bamako, Mali in 2009 after fuel exhaustion.

The MA arrived at Nouakchott at 1202Z. The MC had diplomatic clearance to on-load 4000 liters of fuel. Although two fuel trucks arrived, the mishap pilots determined fuel was not necessary and the MAC decided not to refuel. The MA departed at 1229Z with a target passenger transfer time of 1430Z. This gave the MC two hours to reach a destination that was two hours and 35 minutes away (using a normal DHC-8/Q200 profile). The MC climbed to 24,000 feet (ft). During climbout the MC had indications of a fuel shortage. At 1323Z, the MC climbed to 25,000 ft. At 1341Z, 280 miles prior to Bamako, the MC began a descent. At 1435Z, 105 miles prior to Bamako, the MC leveled off at 6,000 ft. At 1452Z, the MAC directed the MCP to divert northeast to Kolokani, an airstrip 12 miles closer than Bamako. He also directed a gradual descent. At 1509Z, the #2 (right) engine shut down due to fuel exhaustion. At 1514Z, 29 seconds prior to impact, the #1 (left) engine began to shut down due to fuel exhaustion.
This accident I'm quoting is beyond stupidity...
I think Doc would have had his claws on it if it was in Canada with a Canadian registered Dash-8 :smt003

Skimming the report, Transport monitored the company's first CRM course where the action was reviewed as a an example of the breakdown of CRM and SOPs with a new fuel supplier.

Presumably, the matter will be integrated into the CRM course as a result of the SMS investigation.

So in your opinion .... considering we now live in an "SMS World," what should TC have done? Nail the pilots? Won't happen ... Perimeter has an approved SMS so TC is essentially "hands off" in these kind of things unless the "violation" was willful or involved gross uncorrected incompetance which has not been addressed by additional/remedial training.

I think the main reason TC was taking a hard look was the WHOLE company was doing this. That is...doing the before start checklist after shutdown and deferring the "fuel" part of it. Then using various reminders to check they had the proper fuel load for departure.

correction just the Dash 8 crews, metro's have much different style of checklists

Thanks for clarifying.....it was my intention was to indicate that the TSB found that this practice was wide spread amongst the Dash 8 crews. :oops:

How does the Metro checklist deal with verifying the fuel load as per the COM?

"Prior to the PIC dispatching the flight, section 6.6.2 of the Perimeter company operations
manual (COM) requires flight crew members to calculate the fuel needed and to verify it has
been loaded. Tools provided to assist pilots with this requirement are operational flight plans to
calculate the required fuel, and the BEFORE START checklist to verify the aircraft fuel quantity."

it occurs a few times, probably to account for less experienced crews, prior to boarding it is supposed to be verified, in the before start checklist and then again once taxing

Now do you think the crew just might have departed without the fuel to save time? I'm a little confused with the wording of the report. Did they depart with 2 hours fuel for a 2hr35min flight, or did they have 2 hours left in their duty day to fly 2hr35min flight??
If it was the latter, their day was done in Mauritania. I took fuel there in the late '90's. It took almost 5 hours from start to finish to fuel a twin Cessna from drums. The local fuellers were trying to pour the avgas from drums, into metal buckets (with no bonding wires) in blowing sand and 40 degree celsius heat. I thought they were either trying to blow up me and my airplane or send me off with enough sand in my tanks to plug my fuel filters. It was quite a fiasco. I was really glad I decided to fuel the day before our departure. I imagine they have improved since then...

I stopped a few times there and Nouakchott was always one of the fastest turn around in West Africa, when we didn't have to overnight.
You basically get the fuel truck ready and parked in front of you while your engines are still spinning down.
Jet fuel though..No Av gas and flight planning done by UAS (United Aviation Services)


About the Us military Dash-8, they departed without refueling, contrary to their SOP.
Which is, I believe, to top up everywhere they stop in Africa due to unreliable Jet fuel availability.
They did so to save time so that their passengers could be on time for their connecting flight.
Thinking that their next stop was much closer than it actually was...Duh!

And when they realized their mistake, they pressed on instead of diverting or turning back :roll:
