I think what he might have been saying (approx 15 years ago), to put oneself in the pilots shoes, what he or she was facing (the circumstances), prior to the accident.

I think what he was trying to say was that most accidents aren't the result of hijinks or screwing around, or outright negligence, but rather the result of a responsible person trying to do their job, who honestly thought at the time that they were doing the right thing. Or were at least trying to the best of their ability to do the right thing.
